1. Right now, do you have chest pain or pressure, fainting or near-fainting, severe or unusual breathlessness, severe dizziness, new weakness/numbness/confusion, or any sudden or severe symptom?

Do you have diabetes or take a medication that can lower blood sugar (e.g. insulin or a sulfonylurea); are you pregnant or recently postpartum; recovering from surgery, illness or injury; or has a clinician limited your exercise or given you a prescribed diet or a heart/kidney/neurological condition?
3. Do you have joint pain, deconditioning, or balance concerns that make walking or standing movement feel unsteady or uncomfortable?
4. Do you feel generally well today and able to move comfortably?
